HVA’s CareCard is an easy way to keep your important medical information available for first responders during an emergency.
Print out the form and keep a copy visible in your home. Carrying a copy in your purse or wallet is also a good idea.
Make your CareCard easy to find:
- Fold the form into thirds for privacy. Keep the HVA CareCard logo facing out so it is easy to spot.
- Keep the CareCard in a prominent place in your house, like a refrigerator door or near the front door – somewhere first responders can easily find it.
We recommend updating your CareCard every six months or when you’ve had a change in your medical history.
